Edit on Nov 17: Thank to Gundamaniac’s tipoff, we has been notified that this piece of information regarding Gundam Unicorn’s head transformation has been deemed as a hoax.
An explanation below by Gundamaniac being quoted on how the real head transformation is performed.
the way how it transforms (just been confirmed in the latest Dengeki Issue) is by rotating the ear plates on the side, and the mask simply followa the upward rotation and slips into the bottom of the helmet.

*blue - Normal mode face
*red - Destroy mode face
Above is a simple chart(showing the side view of the head) explaining the how the head transformation of Gundam Unicorn goes about from Normal mode into Destroy mode.
As the chart shows, the top half of the head opens up, allowing the blue portion(Normal mode face) to collapse down first. Then the red portion(Destroy mode face) will drop down from the top to take up the position left vacant by the red portion(Normal mode face)
A simple and nifty trick to put 2 face in one head and easily transformable at the very same eh?
